COVID Vaccination Consent Form

Parent/Guardian Consent for Minor Patients seeking a COVID vaccination. Please complete the COVID Vaccination Consent Form and bring it with you to your vaccination appointment for patients under the age of 18. 

Vaccination appointments are NOW available at the International Agri-Center in Tulare. Anyone age 12 and older can get vaccinated and walk-ups are welcome! Community COVID vaccination sites in Tulare County. Residents who meet eligibility criteria can now get vaccinated at these locations. Lost Your COVID Vaccination Card? How to Obtain Your COVID-19 Vaccination Record

When you are needing a new verification of vaccination, here is what you need to know. Tulare County Public Health can only provide verification for vaccines they have administered.

1. Check with your provider to request vaccine verification. All providers who administer the vaccines have access to the California Immunization Registry (CAIR) and can confirm your vaccine.

2. You can request your vaccine records (regardless of provider(s)) directly from CAIR. The form can be completed 2-3 weeks after you have been vaccinated.

CAIR2 Form – Authorization to Release Healthcare Information:

The State process requires uploading a copy of a current valid government-issued ID with picture (i.e. current driver’s license). Additionally, the State record you receive will contain ALL immunizations stored in your immunization record in CAIR. They are unable to produce a record that only contains COVID-19 vaccinations.

3. If you received your COVID-19 vaccine at a County of Tulare sponsored event, and are looking for your vaccination record please follow these steps:

  • Please allow at least 2 to 3 weeks after your vaccine is administered to complete this online form

  • Complete this online form to request it to allow the administering site time to report to CAIR.

Adults requesting records for adult children, family, spouses, or significant others: Anyone over the age of 18 is responsible for requesting his or her own record. Please have them submit their own request to avoid any delays in processing time, as requests are processed in the order they are received.

Minors requesting immunization records: Please have your parent or guardian request your records. Any requests submitted by a minor will be denied unless you are an emancipated youth and have court documents to show this.
